Hazleton Area School District

Any HASD student entering grades 7 through 12 next school year 2025-2026 and interested in participating on one of our FALL sports teams, is required to get a physical examination and complete the PIAA CIPPE form prior to the official start date (Monday, August 11, 2025 for all FALL sports except Golf and Football Heat Acclimatization.  Golf and Football official start date will be (Monday, August 4, 2025).  The HASD/PIAA required physical (Section 6) and PIAA CIPPE form (see ATS Student Login Instructions) (Section 1 through 6) completion must be performed online.  LVHN (Lehigh Valley Health Network) is assisting us in completing this process.  They are also offering a FREE sports physical clinic at the High School on Tuesday, June 10, 2025.   Each team has been assigned a time slot and we ask that all athletes sign up under the correct team time slot.  Also, LVHN is also offering FREE physicals at ExpressCARE if you cannot make the clinic at the high school.  Please see instructions below...They will run from May 1, 2025 thru August 10, 2025.
